Photo/Video Release Form

I hereby give permission to Horse Hill Community League to publish photographs/videos of me as well as my child/children participating in activities at any Horse Hill Community League events/programs.

Photographs/videos may be used to showcase past events/programs, advertise upcoming events/programs, or to generate community engagement.

I acknowledge that these photographs/videos may be published by Horse Hill Community League accounts on several social media sites, Horse Hill Community League website and/or printed material. I understand that I will not be paid for these photographs/videos and have no rights to them. I hereby waive any rights to inspect or approve the finished photograph or advertising copy.

I release Horse Hill Community League from all claims of harm and liability as a result of any distortion, blurring, or alteration, optical illusion, or use in composite form, either intentionally or otherwise which may occur from making, showing, using or distributing these photographs/videos.
